Advisory Subcommittees
The Department of Healthcare and Family Services has established five advisory subcommittees to provide input into the development, implementation and management of Illinois Health Connect (PCCM) and Your Healthcare Plus (Disease Management or DM) programs. Participation in the subcommittees is voluntary and open to any consumer or health care professional who wishes to participate.
The subcommittees meet monthly to quarterly primarily via teleconference. A charter describing the subcommittees, minutes, chairperson and contact information for each subcommittee can be found by clicking on the name of the subcommittee.
