Most Illinois Medicaid clients must pick a health plan and a primary care provider (PCP) for their medical home.
Mandatory Counties:
Clients living in counties where mandatory managed care has been implemented will pick a PCP in a managed care organization (MCO), an Accountable Care Entity (ACE) or a Care Coordination Entity (CCE). Not all plans are available in all counties. The mandatory counties are:
Boone, Champaign, Christian, Clinton, Cook, DeWitt, DuPage, Ford, Henry, Kane, Kankakee, Knox, Lake, Logan, Macon, Madison, McHenry, McLean, Menard, Mercer, Peoria, Piatt, Rock Island, Sangamon, Stark, Tazewell, St. Clair, Vermilion, Will, and Winnebago (30)

Voluntary Counties – Illinois Health Connect Only:
Client living in counties where mandatory managed care has NOT been implemented (voluntary counties), and the primary care case management program is the only option, will pick a PCP in the Illinois Health Connect program. These counties are:

Voluntary Counties – Illinois Health Connect or a voluntary MCO
Some voluntary counties have health plan options (MCOs) in addition to Illinois Health Connect, where clients can choose a PCP in the Illinois Health Connect program or in a voluntary MCO. The voluntary counties where clients have more than one option are:
Adams, Brown, DeKalb, Henderson, Jackson, Lee, Livingston, Perry, Pike, Randolph, Scott, Warren, Washington, Williamson, and Woodford
